RECOVERY AT HOME
We help patients recover at home so they can live as independently as possible after a hospital stay. Home health also plays an important role in helping people avoid going to the hospital in the first place or Nursing Home centers.
HOME HEALTH SERVICES
The care we deliver is personalized to each patient. Your caregiver collaborates closely with your physician to deliver a clear picture of the patient’s progress. We can help manage medications and provide personalized balance programs to help prevent falls and promote safety in the home, among other services.

HOW MANY HOURS PER WEEK WILL I NEED SERVICE?
Our team can provide service for just a few hours a day, or 24 hours a day. We offer a range of services to fit your needs or your loved ones’.
WHAT IF I NEED NON-MEDICAL PERSONAL CARE?
If you need additional assistance with activities of daily living that are non-medical — such as bathing, light housekeeping, meal preparation, companionship, and transportation — our professionals can help. Contact us for a consultation.
HOW TO GET STARTED WITH OUR SERVICES?
After the initial phone call, we will send one of our service coordinators for a free consultation. We will identify your general needs of care and we will explain any questions you may have about our home health services. After this visit, we will schedule an assessment with a registered nurse in which specific areas of care will be identified and a personalized and comprehensive plan will be created. Results of assessment and treatment plan, as well as general recommendations, will be later discussed and provided to family members before starting treatment.
WHEN CAN YOU START CARE?
In most instances, we can start care within 24 hours of your request for service. In many cases, we begin care sooner, if necessary. At C&A we understand the need for care can be immediate and we will address you or your loved one’s needs as soon as possible.

C&A Home Health is licensed by the Agency of Health Care Administration (AHCA) in the state of Florida, certified by Medicare and accredited by the Accreditation Commission for Health Care (ACHC) in the State of Florida.
